Potions
Potions and Decoctions are items made in Alchemy that can be purchased or crafted for use in The Witcher 3: Wild Hunt. They can be used to varying effect to enhance Geralt's abilities in combat.
Potions also have a negative effect on Geralt. Each potion he concocts has a certain level of toxicity to it -- meaning that if Geralt drinks too many potions at once, he will become sick and begin to lose health. He can cure the toxicity by meditating or taking White Honey.
A Witcher can make use of powerful potions that would be lethal to a normal human. These specialty potions can do everything from granting night vision, increased regenerative capabilities, and more. Once a potion is crafted, it can be replenished at any time by meditating with a pure alcohol in your inventory.
Potions, like witcher gear, can be upgraded. The upgrade usually extends the potion's duration and adds a extra dose. The diagrams for potions can be bought at alchemy stores or from herbalists. However some superior diagrams can only be found in random chests throughout the game.

Decoctions are special potions that use monster Mutagens to give Geralt some of his slain enemies' powers (see: Mutagen Locations). They cannot be upgraded. They are found via torn-out pages that can be purchased or discovered -- or learned by reading books.
Note that Nekker Pheromones and Bear Pheromones are mentioned in the in-game texts, but the recipes do not actually exist in the game. However, Drowner Pheromones can be acquired.
